Who controls GlobalPatientRecord™ and who is protecting the privacy of my personal
health data?
The privacy of GlobalPatientRecord™ will be overseen by CareData Patient
Tracking Systems Corporation. Your health information is securely stored on the
CareData network, which cannot be sold or provided to any third parties without
your permission. Your medical information is available to you via the Internet,
but highly secure; just as online banking and online stock accounts are
available via the Internet but secure and only accessible by you using a user
ID and password. Each member account has multiple encryption to protect your
record at all times.

You have total control on who has access to your GlobalPatientRecord™. Only you provide access privileges to doctors, family members or emergency personnel. You can see a list of anyone who has accessed your GlobalPatientRecord™ so that you can
monitor its use.

Who should be a member of the GlobalPatientRecord™?
Absolutely everyone should have a GlobalPatientRecord™. While you or a family
member may not have major medical conditions, there is basic information that
should be accessible for any patient at any time. This would include insurance
information, allergies, medications and dosages, vaccinations, emergency
contact information. In addition, the GlobalPatientRecord™ provides health and
wellness tips and reminders. If you or a family member takes medication or has medical

What if I don't fill out all the information right now? How can I update or make
changes to GlobalPatientRecord™?
Once you have created your personal Health Record, you can make any changes or
updates as needed. You have complete control to your record at all times.
For GlobalPatientRecord™ to work most effectively, you must provide complete
information. Your GlobalPatientRecord™ will send you a quarterly reminder to
keep your record current.
How does this information get to those who need it and when they need it, such as
in the case of an emergency?
You control access to your GlobalPatientRecord™, and decide who is permitted to
view your record, and for what period of time that access is granted. Your
authorization will continue until you revoke it. It is your responsibility to
maintain and update access privileges for your GlobalPatientRecord™. You are encouraged to identify a trusted third party (a personal representative) - with whom to share your GlobalPatientRecord™ access information (User ID and Password), so that he or she can access your health information in the event of a medical emergency during which access to this
information could be important. If possible, this person should be the same
individual identified as your emergency contact in your GlobalPatientRecord™.
Your personal representative is someone you authorize to have access to your
personal health information. No one other than yourself is permitted to make
changes or updates to your GlobalPatientRecord™ accept your personal
representative which has been granted that privilege.
CareData Patient Tracking Systems Corporation makes a wallet card available for
printing, which will present your basic information (assuming you provide it),
including emergency contact, blood type, and allergies. You should keep a
current copy of this wallet card with you at all times, and also share one with
your emergency contact, to facilitate access to important information in your
health record should the need arise.
There are three ways in which your information is accessed in case of an
emergency.

You grant your provider access to GlobalPatientRecord™.

You can grant our GlobalPatientRecord™ Hotline as your emergency point of contact where as you call our hotline 24/7 to provide this information.
Typically, your provider or emergency contact would go to
www.GlobalPatientRecord™.com and be able to access your record by signing in
with an access code you setup. This information would also be instructed on
your GlobalPatientRecord™ wallet card on how to access this information along
with our 24/7 hotline number.
You are encouraged to identify a trusted third party to whom you grant
privileges to your GlobalPatientRecord™ to access information. In the event of a
medical emergency, this information could live saving. If possible, this
person should be the same individual identified as your emergency contact in
your GlobalPatientRecord™.

Should I print a copy of the GlobalPatientRecord™?
It is not important to print the record because it will be continuously
updated and the most accurate version would be the online record. If you still
want to have a paper copy, you can print the entire GlobalPatientRecord™ or just
selected parts of it. However, you are encouraged to print the
GlobalPatientRecord Wallet Card to keep with you at all times, which contains
emergency contact and medical information.

How does information get into the GlobalPatientRecord™?
Do my office staff or I have to fill out the GlobalPatientRecord™ for our
patients? How reliable is patient-provided healthcare information?
Most patients enter their own data into their GlobalPatientRecord™, though
CareData Patient Tracking Systems Corporation is actively creating interfaces
to EMRs and health plans. But in ALL cases the patient owns and controls the
data in their own GlobalPatientRecord™, as well as controlling who has access to
their record. Physician office is under no responsibility to complete or update
a patient's GlobalPatientRecord™; though completing the record may result in
direct payments from pay-for-performance programs. As for the validity of
patient-entered data, the reality of the US outpatient healthcare environment
is that most care is based upon patient- provided information- usually hand
written by the patient using a clipboard. And much of the patient information
is only known by the patient, such as over the counter medications, food
allergies, herbal and food supplements, childhood illnesses, etc. Patients who
can enter this information on their own time without feeling rushed in the
privacy of their own home actually take the time to collect the necessary
information which is more accurate than trying to remember it from the clinic
itself.
What happens if the patient has a GlobalPatientRecord™ and I don't have access to
it?
The existence of an online personal health record in no way diminishes the need
for the patient to provide information directly to the treating physician. The
GlobalPatientRecord™ service reminds patients of this responsibility on the data
input screen of the GlobalPatientRecord™ service and in the overall
GlobalPatientRecord™ terms of service.
How can my office & other physician's offices view a patient's
GlobalPatientRecord™?
Physicians and their approved staff can view the GlobalPatientRecord™s of any
patients as long as they have granted you access to the record. If you use CareData™ then all your patients are already stored in the GlobalPatientRecord™ database for your access.
How does this information get to a hospital ER in an emergency?
The GlobalPatientRecord™ comes with a wallet card, which will present the
patient's basic information including emergency contact, blood type, allergies
and instructions for accessing the record in an emergency. The patient should
keep a current copy of this wallet card with the patient at all times, and also
share one with the patient emergency contact, to facilitate access to important
information in the patient health record should the need arise. The hospital
can access the patients GlobalPatientRecord™ by giving them the code to view it.
Patients are encouraged to identify a trusted third party with whom to share
access information (User ID and Password), in the event of a medical emergency
during which access to this information could be important. If possible, this
person should be the same individual identified as the patient emergency
contact in the GlobalPatientRecord™.
Will the GlobalPatientRecord™ take time away from our duties?
There is no need for anyone in the office to spend any time related to this
service. GlobalPatientRecord™ is in place to save you time and help provide you
with more accurate information at the time of visit by having the patient fill
this out prior to coming to the office. This is great for new and existing
patients. There is less waiting time in the office for the patient and the
information will be updated in your database instantly as the patient updates
their records.
Can my office use a printed copy of the GlobalPatientRecord™ as a Patient
Registration form?
You are encouraged to use the GlobalPatientRecord™ as a registration form for a
patient to fill out ahead of an office visit. GlobalPatientRecord™ works with the CareData EMR/EHR. GlobalPatientRecord™ can also integrate with other EMRs.
Do I increase my liability by using the GlobalPatientRecord™?
No. In fact, leading professional liability carriers are endorsing and
promoting the GlobalPatientRecord™ as a means to improve patient safety, improve
documentation and lower medical liability. Guidelines have been created for the
use of online tools to communicate with patients; including the provision of
personal health records. These can be obtained directly from most liability
carriers or online from various sources. The contractual service language and
other disclaimers suggested by such experts are integrated into the
GlobalPatientRecord™ service and patients must agree to these in order to access
the GlobalPatientRecord™. Most liability carriers promote the concept of online
